Lifestyle Support Worker - Brisbane Northern Suburbs and Sunshine Coast


Synapse is dedicated to reconnecting the lives of those affected by neurocognitive disability.


We believe that everyone affected by neurocognitive disability is entitled to a life of quality, based on their own decisions and choices.


Synapse is currently seeking engaged, pro-active and motivated people to support our clients and make a real difference to their lives.  


The key responsibilities of this role are to;

-        provide support, creating individualised plans and enabling tenants to build skills and confidence to function in the community

-        work with tenants using a variety of measures including written and observational assessments

-        enable clients in preparing healthy meals, cleaning, budgeting, shopping preparation and social interactions

-        work collaboratively, cooperatively and effectively within a team, and with clients, their families/carers, and other stakeholders.


To be successful in this role, you will have;

-        experience in working with people a neurocognitive disability

-        demonstrated capacity to relate to people with a profound disability, their families/guardian, and other stakeholders (this includes government agencies and community services) with dignity and respect

-        ability to assess risk and follow relevant protocols to manage crisis situations calmly and effectively

-        ability to think and act calmly and deal sensitively with people in distress

-        demonstrated effective listening and communication skills in both written and verbal formats. Ability to tailor communication to suit the audience.

-        the ability to work with clients and link them in with other relevant community resources and to promote the choice and self-determination

-        ability to work as part of a team in the provision of care to people with a profound disability

-        knowledge and experience working with Restrictive Practices and Positive Behaviour Support Plans.
