The aim of the service is to increase the health, well-being, and independence of the individual and family upon discharge from hospital. The purpose of the Family Liaison Officer is to build relationships with family and provide continued support post hospital discharge. The service will be delivered initially within the hospital setting with face to face meetings. The Liaison Officer will also provide a follow-up phone call to the individual, or family members six weeks post discharge. Our Liaison Officer will be an ongoing point of contact for individuals and families to provide information, support, and referral to services in their area. This program has been made possible through generous donations from family and friends who have been affected by stroke and brain injury. Please don't hesitate to contact us or fill in an enquiry form today. If you would like to donate to support this initiative, please click on the purple donate link to the left of this page.
