Accommodation Synapse Options Aboriginal & Torres Strait Islander Support Training Client Services Information and Referral Assessment and Planning Support Networks Ask a Legal Question New South Wales Services Stroke Support Services Brain Injury Family Liaison Service Research and Development Publications National Disability Insurance Scheme Brain Injury Family Liaison Service Synapse's Brain Injury Family Liaison Service will provide early support and information to families and individuals who are affected by brain injury, stroke, or neurocognitive disorders. The aim of the service is to increase the health, well-being, and independence of the individual and family upon discharge from hospital. The purpose of the Family Liaison Officer is to build relationships with family and provide continued support post hospital discharge. The service will be delivered initially within the hospital setting with face to face meetings. The Liaison Officer will also provide a follow-up phone call to the individual, or family members six weeks post discharge. Our Liaison Officer will be an ongoing point of contact for individuals and families to provide information, support, and referral to services in their area.
